1 - Do I have a dead [bottom A key] light?
2 - Is that normal guide light behavior when octave shifting?

Well, for question 2, without context, it's hard to say. At the maximum highest octave the last 5 notes don't have a blue light. That is when no instrument is loaded.
One thing you could do to confirm 100% is create a midi template in Komplete Kontrol and assign a color to that key, if it never appears then seems like a faulty led or a connection issue
